Approval of Project/Thesis/Dissertation Forms

This academic form, which is available on the College of Graduate Studies' website and on the Graduate tab of the myUT portal, is required of all graduate students completing a project, thesis, or dissertation requirement.  It must be completed by typing into the PDF form, and can be signed electronically. The form should be completed and signed by all except the Dean of the College of Graduate Studies. Our office obtains the dean's signature. The form should be emailed to GCAcademicSvcs@utoledo.edu by the end of the term. If it is sent after the term ends, it can cause delay in graduation processing.
